Emergency and Critical Care Integration (in and out the facility)-Trial Project for Connecting Long-term Care Services
- Category:Smart Healthcare
Status:Complete Taiwan has entered an aging society, making it important to have a hierarchical medical care system. Patients with acute and severe illnesses receive different types of long-term care services based on their degree of recovery and individual needs after being discharged from hospitals. They will receive continuous and comprehensive nursing after recovery, rehabilitation, and care, avoiding the circumstances where they occupy hospital beds or are repeatedly admitted to hospitals after being discharged so that medical resources can be used effectively and fairly, and this is an important development for smart health policies. In this project, the Zhongxing branch of Taipei City Hospital works with United Financial Intelligence to promote the "Emergency and critical care integration (in and out the facility)-Trial project for connecting long-term care services" for Taipei Community Elderly Long-Term Care Development Association and its members to offer accommodation. Smart dispatching using the "UFI Long-Term Care Map" helps patients who are discharged from hospitals to quickly and conveniently get referred to long-term care institutions in Taipei and find local and suitable resources based on the personalized care conditions they fill in online. The "UFI Home Service Network" offers smart care and monitoring of patients' recovery conditions during home care, with the goal to help patients who are discharged from hospitals quickly find long-term care institutions and services that meet their needs. They can receive comprehensive care after being discharged from hospitals and improve their recovery, and the implementation realizes the vision of incorporating care services to the phases after discharge from hospitals.
